Welcome to Rossett Sports Centre! We hope you are as excited as we are to get going with your EPIC EVENT!Below you will find our Epic Event terms of participation, along with a signature survey to be completed before your booking commences.
DisclaimerTerms of ParticipationThese are the terms of participation that you must read and understand and sign the indicated external form to indicate your acceptance in the above-mentioned activity and its associated games. You as the lead contact will sign the form below, but each member of the group shall also sign to indicate acceptance on the signature survey. All signatures indicate that participation is voluntary and at the individual's choice and risk. If any individual is not willing to accept the risk associated with participation, then they should not take part.Rossett School/ Rossett Sports Centre/Epic Events and the event coordinator(s)/ members of Rossett Sports Centre staff shall not be liable for any loss, damage or injury to any participants or to the personal property of any participant, who enter the event venue.[Each member of the group accepts that their signature warrants that they are not suffering from physical disability or illness whether or not such disability or illness is or may be affected by exercise of whatever degree and that after the event Rossett School/ Rossett Sports Centre/Epic Events and the event coordinator(s)/ members of Rossett Sports Centre staff are indemnified in respect of any disability or illness suffered.]Each participant agrees to comply with all instructions given to me and use any equipment as directed so as not to hurt or injure anyone.All events are run so as to avoid injury or damage to property, and you must comply with all instructions given to you, if you do not follow or ignore the instructions of the event coordinator(s) you may suffer injury or damage to property for which Rossett School/ Rossett Sports Centre/Epic Events and the event coordinator(s)/ members of Rossett Sports Centre staff will not be liable for.Event coordinators and venue management reserve the right to refuse participation/entry if you do not comply with these terms.Nothing in this form shall be construed as limiting or Rossett School/ Rossett Sports Centre/Epic Events and the event coordinator(s)/ members of Rossett Sports Centre staff liability for (a) death or personal injury, which arises as a result of Rossett School/ Rossett Sports Centre/Epic Events and the event coordinator(s)/ members of Rossett Sports Centre staff negligence or the negligence of the event coordinator(s); (b) fraudulent misrepresentation; or (c) any other matter for which it would be illegal or unlawful for Rossett School/ Rossett Sports Centre/Epic Events and the event coordinator(s)/ members of Rossett Sports Centre staff to exclude or attempt to exclude its liability.Groups must arrive at the time that has been notified to you for the session and in the event, a group arrives later you may not be able to participate in the event or may not have the benefit of the allocated time slot which will, in all circumstances, finish promptly.For any reason Rossett Sports Centre/ Rossett School cannot provide you with the event you have booked (the event printed above in the activity field) but offer you an alternative event from their portfolio and you choose to participate in this alternative event, you will not be entitled to any form of refund and or compensation.Please indicate if you have any of the following ailments or conditions:1. Angina/breathing difficulties
2. Dizzy Spells or Fainting
3. Been advised by a doctor that they have a heart condition
4. Blood Circulation Problems
5. Epilepsy or Seizures
6. Pregnant or have been pregnant in the last 6 months
7. Asthma
8. High Blood Pressure
9. Had any operations in the past 2 years
10. Back Problems
11. Joint Problems
12. Broken any bones in the past 2 years
13. Chest Pains
14. Claustrophobia
15. IntoxicatedFor any participants under the age of 18:I confirm that the persons named within the signature survey and group booking (please tick to indicate under 18’s) can participate in Rossett School/ Rossett Sports Centre event. I also understand that while the event coordinators will take every precaution to ensure that accidents do not happen, they cannot necessarily be held responsible for any loss, damage or injury suffered to any person under the age of 18.Rossett School/ Rossett Sports Centre will comply with its obligations under the data protection legislation and not share your personal details with any 3rd party.To show your agreement to these terms, before the commencement of your booking, please sign the external form.
